If you have read about Radiesse, you have probably read about it as a structural filler that builds projection in the chin or jawline. Bio-Stimulation with Radiesse is a different use of the same product, with a completely different purpose.
Instead of placing the product deep against the bone for projection, we dilute it with saline and place it across larger areas as a series of small, threaded passes just under the skin. The microspheres still do what they do best, which is signal your skin to build collagen and elastin around them. The result is not added volume. It is firmer, smoother, more elastic skin in the area we treated.
This is the treatment for clients who like the structural results of Radiesse but who want it for skin quality, not for shaping. It is also one of the best non-laser options we offer for crepey skin on the neck, the décolletage, the upper arms, and the backs of the hands.
What makes Bio-Stimulation with Radiesse different
The product itself is the same FDA-cleared Radiesse you would get for chin or jawline work. What changes is the dilution, the depth, and the area. A few specifics about how we approach bio-stimulation at ABL:
- Hyper-dilution is the technique. Standard Radiesse is mixed with saline (and a small amount of lidocaine) to thin it into a more spreadable consistency. The microspheres do the bio-stim work; the gel carrier just helps them sit evenly under the skin.
- Threading, not bolus. Where structural Radiesse is placed as deeper deposits, bio-stim Radiesse is delivered through small fan-like threads that distribute the product across the area being treated. A cannula is the common tool here, which means fewer entry points and less bruising.
- Series, not single session. Bio-stimulation works through gradual collagen formation, so a single visit is not the goal. Most clients do 2 to 3 sessions spaced 4 to 6 weeks apart, then maintenance once a year.
- Specific to your area. The neck, décolletage, hands, upper arms, knees, and lower face all respond to bio-stim, but each area has its own technique. Gaby walks through which areas are right for you and what to expect in each.

Where Bio-Stimulation with Radiesse works best
These are the areas we treat most often:
- Neck and décolletage. Crepey skin and “tech neck” lines respond well. This is one of our most popular bio-stim use cases.

- Upper arms. The “bat wing” texture along the inner upper arm responds slowly but visibly. This area takes 3 sessions to show clearly.
- Knees. The skin above the knee gets crepey before the surrounding tissue does. Bio-stim is one of the few non-laser options that addresses this.
- Lower face and jawline (skin layer). Paired with structural jawline Radiesse, bio-stim tightens the surface around the new frame.
If your concern is structural volume loss (a hollow cheek, a recessed chin, a soft jawline), bio-stim is not the right tool. The conversation at the consult will sort out which approach matches your goal.
What to expect at your bio-stimulation session
Your first visit is a free 20-minute consultation. Gaby looks at the areas you want addressed, takes photos for the before/after series, asks about your skin history, and lays out a plan: how many areas, how many sessions, what the spacing looks like, and what the total cost will be.
Treatment can usually happen on the same visit if the schedule allows.
On treatment day:
- Numbing cream goes on the treatment area for 20 to 30 minutes. Larger areas (full neck, full décolletage) need more numbing time.
- Radiesse is mixed with saline and a small amount of lidocaine in the syringe to create the diluted formulation.
- Injections are placed with a cannula (most areas) or a small needle (some areas). Cannula entry points are minimal, usually one or two per area.
- The product is laid down in a fan or thread pattern just under the skin so it covers the full area evenly.
- Treatment time depends on the area. The backs of the hands take 15 to 20 minutes. The full neck and décolletage take 45 to 60 minutes.
- You may see a subtle tightening immediately, mostly from swelling. The true result comes in over weeks as your skin builds collagen around the microspheres.
- Most clients return to normal activities the same day. Mild redness or pinpoint bruising at entry sites can last 2 to 4 days.
Your second session is scheduled 4 to 6 weeks out. Most clients see a clear difference at that visit, and the third session (4 to 6 weeks after the second) tends to lock in the result.
Aftercare for bio-stimulation
Bio-stim aftercare is light. The main points:
- First 24 hours: avoid pressure or massage on the treated area, no strenuous exercise, no saunas, no hot yoga, no alcohol.
- Optional gentle massage starting day 2 (only if Gaby instructs). For larger areas, light massage at home for the first week can help the product distribute evenly. Wait for instruction at the end of your session before doing this.
- Sun protection. SPF 30 or higher on treated areas helps the collagen response and protects against pigment changes during the healing phase. This is a 6-week habit, not a 1-day habit.
- Hydration matters. Drink water. Collagen formation is partly a hydration story.
- What to expect over the weeks: the area may feel a little tight or slightly firmer than usual for the first 2 to 3 weeks. That is the product working. Visible firmness comes in around weeks 6 to 12 and continues building through month 6.
